Office Policies and Procedures
You are required to still wear a mask in our office. Please ensure you are prepared to bring your mask, health card, and list of medications to every appointment.
Missed appointment/ Cancellation less than 24 hours-
Your scheduled appointment time is reserved just for you. Our providers make every effort to be on time for scheduled appointments. In return, we ask that you help us by keeping your scheduled appointments and by notifying us in advance when you are unable to do so. When you give us advanced notice we are able to accommodate other patients. A missed appointment is when you fail to show for a scheduled appointment without notifying the clinic or when you cancel without sufficient notice.
If you are unable to attend your scheduled appointment, contact our clinic immediately. Since appointments are in high demand, your prompt cancellation will give another patient the opportunity to access timely medical care. Appointments that are not cancelled one (1) business day prior to your scheduled appointment (or by 12 pm on Sunday for appointments taking place the following Monday) will be subject to fees. The fee will be $50.00 and will need to be paid prior to re booking any appointment. This policy ensures that we can continue to work diligently to get to all appointments in a timely manner.
There is no charge for rescheduling or cancelling an appointment if we are notified with proper notice. Fees may be waived due to family or health emergencies at the discretion of the physician and staff.
Late Arrival-
Our providers and staff aim to make your visit a pleasant one. In our efforts to make your visit more comfortable and to minimize your wait time, our office has implemented a late arrival policy. If a patient is more than 15 minutes late for an appointment, the appointment may need to be rescheduled. This is to ensure that the patients who arrive on time do not wait longer than necessary to see the provider. You may be given the option to wait for another appointment time on the same day if one is available. We will try to accommodate lateācomers as best as possible, but cannot compromise on the quality and timely care provided to our other patients. All patients are encouraged to arrive at the office at least 10 minutes prior to the scheduled appointment to register and complete any required paperwork. We appreciate your compliance and understanding with this policy so that we can continue to provide excellent medical care as well as excellent customer service.
At Appointment- Due to high demand of appointments we ask that you keep your appointment to (1) one major or (2) two minor issues at a time. If you require anything more we will gladly book another appointment to discuss further issues. When having a telephone appointment for prescription renewals that appointment is ONLY for the prescriptions that are needing to be renewed. Anything further we will book another appointment. This will ensure that we stay on schedule to continue to provide excellent medical care.
Prescription renewals: As of January 1st 2023, all prescriptions require an appointment. All patients are required to keep track of the prescriptions and to notify us with at least 2 weeks notice prior to running out. There is a $20.00 charge when needing a prescription without having an appointment. This fee also applies if you do not give enough notice for an appointment to be made.
Scent Free: Our office is a scent free zone. Please refrain from wearing perfumes/colognes and any highly scented deodorant to our office. We have many patients/staff with allergies that can not handle the strong scents.
Food/Drink Policy: Our office has a strict no food or drink policy. Due to the nature of Patient/Staff allergies food and drinks are not permitted. Please finish them prior to coming into the office or leaving them in your vehicle.
ZERO Tolerance-
We have noticed an increase in aggressive behaviour and harrassment. Our office has a ZERO tolerance policy on abuse or aggressive behaviour in the office or by telephone. Please be patient with our staff, they are working hard to assist you in a timely manner.
Any bullying, disrespectful, abusive or rude behaviour will result in you being asked to leave, and risk being discharged from the practice.
We appreciate your understanding.
Dr. S. Ansari and Staff.
